YMCA Childcare- Strandwood

Based on interviews, the licensee did not comply with the section cited above when C1 was left unsupervisved for about 2-5 minutes and observed crying and upset which poses an immediate risk to the health, safety or personal rights of children in care.

A REVIEW OF RECORDS AND INTERVIEWS WHICH REVEALED A CHILD WAS ALLOWED TO WALK OUTSIDE TO THE PLAYGROUND WITH ANOTHER CHILD'S FAMILY The facility has terminated employment of the employee that allowed the child to walk out with another child's family, and implemented a new policy to station only employees that are familiar with the children in care to be at the door during transition. Licensee will submit a summary of the changes the facility made for transition times to prevent further incidents by 9/25/24.

Outlet has been permanently removed. Director has submitted all documentation for lead testing. This deficiency was cleared during the inspection. Letter of Clearance was provided. Per LPA's inspection there is at least one outlet in the kitchen area that exceeded action level established by State for lead exposure. Children used the outlet for drinking water. This posed a potential risk to health safety of children.
