101229(a)(1) Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time. This requirement was not met evidenced by: 1 The facility submitted a restroom protocols review on 2/23/2026 was signed by all staff members stating five step instructions when escorting children to and from the bathroom. Based on interviews and record review, Staff one (S1) left Child one (C1) without supervison. 8

The statement will be sent to our office by the due date of 5/18/2021. The director will also write a plan to prevent of happening such Based on self report, interviews with staff and one child, a 6 year old child left the classroom for few minutes and a witness brought the child back. The facility failed to meet the above Regulations. This is an immediate risk to the health and safety of children in care. 8 incident in the future. An immediate civil penalty of 500.00 is hereby assessed for absence of supervision for today and 100.00 per day until corrected.
