If food is being brought in from outside, facility will check the food prior to serving Through interviews, it was revealed lunch provided from the school district, was not checked by staff prior to serving. Licensee states they will ensure all children are wearing appropriate footwear while outside at all times Through interviews, it was alleged that children are not wearing shoes while outside which has caused an increase in injuries. This poses an immediate health, safety or personal rights risk to children in care.
Faciity will meet with staff individually to discuss appropiate ways to talk to children, including during difficult situations. Director will submit signed meeting notes from each staff Through interviews, it was revealed that staff are yelling at children to enforce rules, or at times they feel the children are not listening to teachers, and/or rules This poses an immediate health, safety or personal rights risk to children in care.
Facility will discuss with staff other ways to communicate with staff during business hours, which may include the use of walkie talkies. Director will also meet with staff to discuss personal cell phone usage. Director will submit a cell phone plan signed by staff to LPA Through interviews it was revealed that teachers are using cell phones while supervising children, either for work or personal purposes, causing a lack of supervision This poses an immediate health, safety or personal rights risk to children in care.
Facility will conduct a staff meeting with all staff regarding reporting requirements to parents of injuries that occur that are more significant than a minor cut or scratch. This includes timeline of reporting to parents. Facility will submit meeting notes with staff signatures to LPA by 7/7/23 Facility failed to report head injuries to parents at time injury occurred. This poses a potential health, safety, or personal rights risk to children in care.
Facility will conduct a staff meeting with all staff regarding the proper way to change children's diaper in each area of the facility, including while outside. Facility will submit meeting notes with staff signatures to LPA by 7/7/23 Facility did not change a child's diaper with proper furnishings or allow privacy This poses a potential health, safety, or personal rights risk to children in care.
Based on record review and interview, the two (2) staff members were not fully cleared nor associated to the facility which poses an immediate risk to children in care.
Based on interviews conducted and a review of records it was determined that C1 wandered approximately 55ft away from staff and was left unattended for a period of time. This is an immediate health and safety risk to children in care.
Based on interviews conducted it was revealed that one teacher and one aide were outside with approximately 30 children for a period of time. This is an immediate health and safety risk to children in care.
