Based on observation, the licensee did not comply with the section cited above in that the climbing structure and two climbable dinosaurs did not have sufficient cushioning material to absorbs falls and the wooden boarder around climber and sandbox were in disrepair with chipped paint and decayed wooden supports which poses a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above for out of 2 out of 3 staff present. S2 and S3 were working in the facility without a criminal record clearance with Community Care Licensing which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in two out of two files. S1 and S2 did not have proof of completion of the Mandated Reporter Training on file as required, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in one out of two files reviewed. S2 did not have proof of immunity to Measles or Pertussis which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in one out of two files reviewed. S2 did not have a health screening with TB clearance on file which poses/posed a potential health, safety or personal rights risk to persons in care.
