(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met as evidence by: Director agrees to remove all affected outlets from service. Director agrees maintain to their contract with a bottled water provider to provide safe water for consumption, and food preparation. Director plans to request a waiver to provide alternate portable water. Director agrees to update the plan of operation for the facility By observation from LPA Gibson water fountains tested A and B still had water accessible to children, employees, and visitors 8 and inform parents of ALE notification. by plan of correction date of 06/23/22.

Based on observation, the licensee did not comply with the section cited above in disinfectants and cleaning solutions were accessible in a supply closet which posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in that staff were engaged in other activities while supervising napping children, as described in LIC 809, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in one child was present who had not been signed in and nine children were present who had been signed in on the wrong date which poses a potential health, safety or personal rights risk to persons in care.
