Licensee stated it must had been left out by the cleaners this morning. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on LPA staff records review, staff 1 (Mandated Reporter, MMR, education requirements, LIC508, LIC9052, LIC9108 and 2023 flu declination), staff 2 (Mandated Reporter) and staff 4 (TB) are missing required Child Care Licensing forms. The licensee did not comply with the section cited above in [count] out of which poses/posed a potential health, safety or personal rights risk to persons in care.
