Signed meeting notes will be submitted to LPA by POC date CCLD was made aware of the Unusual Incident on 11/14/25, when the incident occurred 11/6/25, which is outside of the 24 hour reporting requirement, which poses a potential health, safety or personal rights risks to children in care
