At the time of the renewal inspection the certification representative observed that the facility did not have an emergency plan.

At the time of the renewal inspection the certification representative observed that rooms 6, 7, 8, and 9 did not have hand washing signs posted that state, "A facility person and an able child shall wash his hands after toileting and before eating."

At the time of the renewal inspection the certification representative observed that the facility did not have documentation of the testing of the fire system within the last 30 days.

At the time of the renewal inspection the certification representative observed that the agreement form for child #1, 2, and 3 was not signed by the parent within the last 6 months.

During the unannounced inspection, certification representative conducted a facility walkthrough, and the indoor play space was being used for storage. There were no equipment or toys in place for large muscle activity which included running, jumping, climbing and riding in the play space

During the unannounced inspection, certification representative reviewed 18 staff files and the file of staff #1 (last 8/30/23) and staff #3 (last 6/30/23) did not include an updated PQAS certified pediatric CPR and first aid training.

During the unannounced inspection, certification representative reviewed 18 staff files and the file for staff #4 did not include a verification of education.

During the unannounced inspection, certification representative reviewed 18 staff files and the file for staff #2 did not include the 10-hr. health and safety training with the 1hr update or the 10-hr health and safety revised or the 2-part 6 hr. health and safety per Announcement C-24-03 Health and Safety training.

During the initial inspection, certification representative reviewed 10 staff files and the file for staff #1, staff #2, staff #3, staff #4 and staff #5 did not include a record of emergency plan training.

During the initial inspection, certification representative reviewed 10 children's files and the file for child #1 did not include an updated health assessment (last date 6/11/24).

During the initial inspection, certification representative reviewed 10 children's files and the health reports for child #2 and child #3 did not include the statement that the child is able to participate in in childcare and appears to be free from contagious or communicable disease.

During the initial inspection, certification representative conducted a facility walkthrough, and the fire extinguisher inspection date has been expired on Jan 2024 in room #3 and in the hallway next to room #4.

During the initial inspection, certification representative reviewed 10 staff files and the file for staff #5, (2/1/24) staff #7, staff #8 and staff #9 did not include a fire safety training or an updated fire safety training.

During the initial inspection, certification representative reviewed 10 staff files and staff #5 file did not include a staff evaluation.

During the initial inspection, certification representative reviewed 10 staff files and the file for staff #6, staff #7, staff #8 and staff #9 did not include a verification of experience prior to service at the facility.
