Certification rep reviewed Staff #1 record and observed staff #1 did not obtain an acceptable FBI DHS clearance.; FBI clearance observed in record was from the Department of Education. Staff #4 did not obtain an FBI and child abuse clearance.

During the renewal inspection cert rep observe the facility did not manually test its fire alarm at least once every 30 days and maintained a written record of testing with the facility's fire drill logs in accordance with 62 P.S. § 1016(c)(1). Durin the renewal inspection staff #5 was unable to properly shut off the fire alarm system.

Certification rep reviewed staff #1 record and observed, staff did not have in record Health and safety training part 2 within 90 days of date of hire.

Cert ep reviewed staff #2, #3 and #4 record and observed it did not include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Certification rep reviewed staff #3 record and observed one written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person was not in record.

Certification rep reviewed staff #1 and #3 record and observed staff did not complete Pediatric First Aid and CPR training within 90 days of date of hire.
