During the walk-through in the main building certification reps observed mouse droppings in the back of the sink area and the window of the toddler classroom, in the pantry closet shelves, alongside of the PreK classroom left wall closets and floors. Certification rep completed a walkthrough of the church building and observed mouse droppings on top of the table of the toddler classroom and right side of wall.

During the complaint investigation certification reps observed a black and mild cigar on top of the shelf in the toddler classroom of the church building.

During the walkthrough certification reps observed rugs, walls, floors, shelves and chairs unclean throughout out the facility in the main building.

During the walk-through certification reps observed children in the pre-k 3 classroom with no proper rest equipment.

During the renewal inspection certification rep reviewed staff #1 record and observe State Police Criminal check request in record is under review for control, date of hire: 10/4/2023.

During the renewal inspection certification rep reviewed staff #2 record and observed no record of completion of the 10 Hour Health and Safety Training.

At the time of unannounced monitoring inspection, file of staff person #1 did not include verification that staff person #1 has completed pre-service training in Pediatric First Aid and CPR from a PQAS-approved trainer and PQAS approved curriculum within 90 days of the date of hire (see LIS code sheet).
