Staff #2 was observed taking a child to the bathroom leaving 5 children unsupervised alone not able to hear, see or access the children's whereabouts. Staff#2 was observed minutes later leaving their area with 6 children unsupervised to go speak with staff #3 in the playspace area not able to hear, see or access the children's whereabouts.
Observed 7/13/2026 Staff person #2 and 3 was observed to be caring for children unsupervised. Staff person #2 and 3 has not completed the following pre-service training required Pediatric First Aid-CPR training prior to caring for children unsupervised. Staff person #1 was observed to be caring for children unsupervised. Staff person #1 does not have current pediatric first aid/CPR training. This is evidenced by the previously documented pediatric first aid/CPR training expiration date of 6/11/2026.
Observed 7/13/2026 staff #1 had 6 children assigned to them, staff #1 was observed in the large space area with 3 children leaving 3 children in computer Area #1 unsupervised alone not able to hear, see or access the children's whereabouts. During lunch time staff #3 had 3 children, during lunch child#1 was observed leaving the lunch area to return to the play space with a tablet. Staff #3 could not hear, see or access the child's whereabouts for four minutes. Staff #4 was observed walking child #2 to the bathroom for toileting, staff #4 left child #2 in the hallway near the bathroom alone unsupervised to attend to another child who was trying to exit the childcare space.
