The Emergency Contact form(s) for child(ren) file(s) #1 lacked the "Address of the Listed Release Person." (SEE LIS CODE SHEET)

According to the Fire Drill Log, the last fire drill conducted was in 12/16/24.

Staff person#1 has not completed mandated reporter training within the last 60 months. This is evidenced by the previously documented mandated reporter training on file being dated 5/2/20.

The NSOR certification for Staff #1 was dated after Staff #1's Date of Hire. Therefore, Staff #1 was incorrectly provisionally hired.

At approximately 3:55 pm, facility person #1 was in the hall with the children outside the restroom preparing to go to the cafeteria. When asked to identify their group, 13 children were identified. The 13 children were all identified as being young school age children, making the ratio 1:12.

The file of facility person #1 contained documentation of the completion of the 1 hour "Pennsylvania Health and Safety Update 2022" being completed on 1/31/2023, which is after the required date of 12/30/2023.

Documentation of current pediatric first aid and pediatric CPR of faciltiy person #2 was renewed after the expiration of the prior (8/2/2021 and 9/18/2023).

The emergency contact form of child #3 did not contain the telephone number of the child's physician.

The emergency contact form of child #3 did not contain the work address and telephone number of the enrolling parent.

The emergency contact form of child #1 did not contain 2 addresses and child #4 did not contain 3 telephone numbers of the persons designated by the parent to whom the child may be released.

Child file #1 contained an initial health assessment dated 11/10/21 which was more than 60 days after admission to the program. Child file #2 contained an initial health assessment dated 10/26/21 which was more than 60 days after admission to the program.

Child file #1 lacked the fee amount on the agreement form.

Child file #2 lacked the parent signature for consent of emergency medical care on the emergency contact form.

Staff file #1 contained documentation of 11 of the 12 clock hours of child care training required annually and borrowed 1 hr from 9/23/22 training. Staff file #2 contained documentation of 3 of the 12 clock hours of child care training required annually and borrowed 9 hrs from 10/2/22 training.

The child file for Child #1 contained an emergency contact information form that lacked an address for one of the designated release persons listed.
