The safety device used to prevent the hot water from turning on at the sink located in the bathroom in the preschool classroom was not secured, which allowed the hot water to be turned on. The hot water temperature reached a temperature of 113° F.

Observed damage to the bathroom wall located in the preschool bathroom.

Observed a lightweight plastic storage shelf in the corner of the one-year-old classroom with was not stable. The shelf, which was between 5 - 6 feet tall presented a tip hazard.

The file for child #2 included a health report which was not updated at least once every 6 months for an infant or young toddler (see LIS code sheet), the most recent health report was dated 01/09/2026 with no subsequent health report on file within 6 months of the previous report.

The file for child #3 included a health report which was not updated at least once every 12 months for an older toddler or preschool child (see LIS code sheet), the most recent health report was dated 12/17/2024 with no subsequent health report on file within 12 months of the previous report.

Lunch provided at the facility did not include at least one grain; lunch included: chicken, mashed potatoes, pears, and milk.

Emergency contact information was not present in the child care space for child #1 while receiving care in the infant classroom; the emergency contact information was taken outside while the rest of the class went outside to play. *Corrected at the time of the inspection.
