During the review of the staff files, staff person #1's file contained a Child Abuse Clearance and a PA State Police clearance that were unacceptable (volunteer clearances).

In reviewing of staff files, staff person #1's file contained a FBI clearance that was late in obtaining (past the 45 days of hire---see LIS code sheet) (FBI clearance dated 2/14/25). In reviewing of staff files, staff person #2's file did not contain a FBI clearance. In reviewing of staff files, staff person #3's file did not contain a NSOR clearance.

In reviewing of staff files, staff person #3's file did not contain verification of completion of Emergency Plan training.

During the physical site inspection, there was not a thermometer present in the refrigerator.

During review of the staff files, staff person #1 and staff person #3's files contained Health Assessments that were late in obtaining (see LIS code sheet) (staff person #1 date of health assessment 1/13/25; staff person #3 date of health assessment 6/24/24).
