At the time of inspection, facility person #1, who is under the age of 18; was observed working as a staff member in the infant classroom.

At the time of inspection, the emergency contact forms for child #2 and child #3 were missing the name, address and telephone number of the child's physician or source of medical care.

At the time of inspection, the emergency contact form for child #3 and child #5 did not have the parent's signature for consent of emergency medical and minor first-aid. The emergency contact form for child #4 did not have the parent's signature for consent for administration of minor first-aid procedures by facility staff.

At the time of inspection, the emergency contact forms for child #1 was missing the addresses of the release persons

At the time of inspection, the file for staff member #2 only included verification of 10 hours of annual professional development training.

At the time of inspection, the operator did not have verification of annual emergency plan training for staff member #1, staff member #2, staff member #3, staff member #4, staff member and staff member #5.

At the time of inspection, a rug observed in the bathroom was in disrepair.

At the time of inspection, nonprescription medication was observed expired and without a medication log identifying the whom the medication is to be administered.

At the time of inspection, the facility did not have verification of the annual staff evaluations for staff members #1.

At the time of inspection, the files for staff members #2 did not contain verification of the annual 12 clock hours of child care training.

During the renewal inspection on 7/21/23 cleaning products were observed in an unlocked cabinet under the sink in the Exploration, Tiger and Lions Classroom.

During the renewal inspection on 7/21/23 the thermometer in the refrigerator in the upstairs kitchen had a reading of 55 degrees F.

During the renewal inspection on 7/21/23 the health assessment for staff person #1 was not signed as required. The health assessment for staff person #2 and #3 did not include TB testing dates.

During the renewal inspection the operator revealed that the facility took a group of school aged children to a swimming pool on one occasion in June of 2022. The facility does not have a certified life guard on staff as required.

During the renewal inspection plastic bags were observed in the lower level of the children's cubbie units in the toddler classroom.

During the renewal inspection on 7/12/2022 the strobe light in the lower level toddlers classroom was not flashing as required during the fire alarm test.

During the renewal inspection on 7/12/022 staff person #1 did not have Pediatric first aid and Pediatric CPR and was observed working alone with the children.
