On 3/14/25 the director reported that a child was left outside on the playground on 3/13/25. Director reported child was outside alone for approximately 5 minutes. The facility was made aware that the child was missing when a head start staff brought the child into the day care.
On 2/8/24 a preschool aged child (child #1) was left in an inside stairwell at the facility for roughly 3 minutes, by staff #1. The preschool children and staff #1 were heading to the outdoor play space when child #1 was left behind. Child #1 was found and returned to the staff at the day care.
While at the facility on 3/8/23 for the renewal inspection the inspector observed the file for staff #1 and noted that this staff obtained an updated PDE FBI clearances versus the DHS FBI that is required.
While at the facility unannounced on 2/24/22 for the renewal inspection the inspector observed the file for staff #1 and noted a first day of working in child care as 8/2/21. This staff had a Pennsylvania Department of Education FBI clearance dated 8/24/18. This staff was required to have a Pa Department of Human Services FBI clearance in order to work in direct child care and not the PDE FBI.
