Observed 1/16/2025 Staff #2 was completing a diaper change and after completing the diaper change staff #2 did not handwash child's hands after being diapered.
Observed 1/16/2025 Staff #1 did not clean the diaper changing surfaces after diapering by wiping the surface with a sanitizing solution or by changing a pad or other surface coverings.
Observed 1/16/2025 staff #1 was missing a tuberculosis screening by the Mantoux method at initial employment.
