The emergency plan training on file for staff person #1 was not updated annually, as required. This is evidenced by the training on file dated 6/30/25.

The fire safety training on file for staff person #1 was not updated annually, as required. This is evidenced by the fire safety training on file dated 6/25/25.

At the time of the inspection, staff person #2 did not have 4 years (5,000 hours) of previous childcare experience on file.

At the time of the inspection, the agreement on file for child #4 did not contain the fee amount to be charged. The emergency contact form for child #5 did not include the address of the person to whom the child may be released.

At the time of the inspection on 12/5/25, there was no documentation of manual monthly monitoring of the fire detection system since the program opened on 9/29/25. This is required to be conducted every 30 days.

At the time of the inspection, staff person #1 only had 9 clock hours of child care training during the 11/2024-11/2025 training period.

On 10/29/25, at 6:50 a.m., Staff #1 was observed with 11 children in care, and no other adults, either staff or facility persons, were present at the facility.

At the time of the inspection, there was no documentation on file for proof of installation of the interconnected fire detection system.

At the time of the inspection, staff person #1 did not updated fire safety training annually. This is evidenced by the current fire safety training on file dated 9/5/25 and the previous training on file dated 4/12/24.
