In the infant room, a mini fridge was observed with a microwave resting on top. The fridge was observed to wobble when touched, causing the microwave to be at risk of falling.
Staff #1 had a State Police clearance dated 10/9/18 and updated 1/5/24, a Child Abuse clearance dated 11/5/18 and updated on 1/5/24, and an FBI clearance dated 11/29/18 and updated on 1/26/24, all of which were updated greater than 60 months. Clearances for staff #1 are now on file.
Fire detection testing was observed to be completed greater than 30 days on the following occasions: 8/11/23-9/11/23, 12/8/23-1/8/24, 1/8/24-2/8/24, and 3/8/24-4/8/24.
Medication (inhaler/albuterol) for Child #1 was observed to be without the original container or accompanying prescription and did not have a complete medication log (observed to be signed and dated but contained no instructions or other details.)
During the renewal inspection, chipped paint was observed on the storage cabinet and on the legs of the teacher's table in the school-age classroom.
During the renewal inspection, the immunization record did not include the influenza vaccine for Child #1. The file for Child #1 did not include a statement of exemption from immunization.
During the renewal inspection, it was verified that the facility serves up to 76 children at any given time with only one group supervisor employed.
On 2/24/22, around 10:15 AM, it was observed that 2 infants were unable to be seen by any staff, as the two infants were sleeping on one side of the room with an accordion divider pulled across the room. Staff person #1, the only staff person in the room, was on the other side of the divider. The divider was pulled back during the inspection, allowing staff person #1 to properly supervise all children in care.
