Cert Rep observed a bottle of Purell Hand Sanitizer on a wooden table, located on the right corner of the stage, in the childcare space. Children had access to this bottle. Cert Rep observed a bottle of Oxivir Five -16 Concentrate, with a warning to keep it out away from children, under the same wooden table, on the right corner of the stage, in the childcare space. Children had access to this bottle.

In the file for Child # 1, Cert Rep observed that the Emergency Contact form was last updated 8/4/24. The Emergency Contact form was not updated in a timely manner.

The facility did not have a written agreement between school and the program's legal entity documenting the school's process for the disposal of hazardous materials and biological contaminants at the time of inspection. The facility director expressed their plans to meet regulation during inspection. *Corrected on Site*

At the time of the renewal inspection follow up visit to verify corrections, the plan of corrections for the violation citing 3270.102c on SIN-00231442 had not been fully implemented. On 11/13/23, the depth of the mulch did not meet the recommendations of the United States Consumer Product Safety Commission. In multiple spots measured near the equipment, the mulch measured between 6 and 8 inches.
