At the time of the inspection, Child #1 had an emergency contact form that was not updated within the last 6-months.
On 6/30/2023 Certification rep verified complaint by completing a walkthrough of the facility during the complaint investigation and observed evidence of mouse droppings by the bedding area back closet and on the toy shelf in the older toddler classroom, in the younger toddler classroom right wall where the basket of soft toys is placed and in both preschool rooms' kitchen area corners.
As a result of the complaint Investigaton conducted on 6/30/2023, it was determined this complaint is verified due to staff #1 stating to certification rep, personal information of a facility employee was disclosed to another facility employee.
Certification rep reviewed the providers emergency plan and observed the "continuity of operations" section is missing in the emergency plan.
Certification reviewed staff #1 and observed staff does not have TB shot information recorded on the physical form. Hired date: 8/28/14. Certification rep reviewed staff #2 file and observed the physical examination form was not complete, the # 2 box was not checked on the physical examination form.
Certification rep reviewed child #1 file and observed the home telephone number, business name and address is missing on the emergency contact form.
