The file for staff #20 did not contain any out of state Florida Clearances. This file also lacked documentation to verify that a DHS FBI Clearance has been requested. The file for Staff # 20 contained a Pennsylvania State Police Clearance, a Child Abuse Clearance and a National Sex Offender Registry Clearance.

The file for staff person #22 did not contain a health assessment that confirmed the individual was suitable to provide childcare and was currently free from any communicable diseases.

The file for staff #21 only contained one written, nonfamilial reference letter. While there were two references in the file, both were written by the same individual and from the same previous employer. The file for staff # 21 still needs at least one more written, nonfamilial reference letter attesting to the individual's suitability to serve as a facility person.

The file for Staff #4 did not contain verification of the childcare work experience amounts required to perform the duties of the position Staff #4 was hired for. The files for Staff #17 & #18 do not contain verification of the educational requirements to perform the duties they were hired for.

Continued non-compliance: The facility owner did not attend the Existing Childcare Provider Training held by the Central Office of Child Development and Early Learning on 4/16/26 as described on the tiered plan of correction. The child file for Child # 1 did not include the Release Person's address, as required.

Cert reps observed that the staff file for Staff #3 did not contain a valid NSOR clearance.

Cert Reps observed that the staff files for Staff members #3, #4, #10, #11, #12, #14, #17 and #18 did not contain a signed Disclosure Statement. Cert Reps also observed that the still file for Staff Person #5's contain an incomplete Disclosure Statement that was not signed by a witness.

Cert Reps observed plastic spray bottles in the Infant, Toddlers 2, Toddlers 1, Twos, Get Set 2, Pre-School 1, Pre-school 2 and Pre-K 1 rooms, that did not include a label listing the specific contents of the bottles.

Cert Reps observed, in the Pre-K2 and Kindergarten room, that the trash cans were not emptied daily.

Cert Reps observed in the Get Set 2 room that there was no liner in the bathroom trash can.

Cert Reps observed in the Gym that the play equipment in the alcove area was significantly dirty and stained. Cert Reps also observed in the Pre-K, Pre-School 1, Pre-School 2 rooms dirt and debris on the floor of several rest equipment storage areas, behind and under several stacks of cots.

Cert reps observed that in the Toddler 2, Toddler 1, Twos, Pre-School 1, Pre-school 2 and Get Set 2 rooms, the rest equipment was not labeled for the use of a specific child.

Cert Reps observed in the Get Set 2 and Pre-K 1 rooms that there were no thermometers in either refrigerator.

Cert Rep Observed that the files for Staff Members #3, #4 and #7 did not include verification that these staff members completed a TB screening at the time of initial employment. Staff Person # 4 does not have a Health Assessment or TB test result on File.

Cert Reps observed that the fire drill log indicated a fire drill was completed 12/3/25 and the next fire drill was completed 2/4/26, which was 63 days later. Fire Drills were not completed timely.

Cert Rep observed that the staff file for Staff # 18 did not include verification that Health and Safety training was completed within 90 days of hire. Cert Reps observed that the staff file for Staff Member #1 contained a Pediatric CPR/First Aid training Certificate, issued 10/13/25, from a PQAS trainer whose training credentials expired 8/26/25. This training certificate is not valid. Cert Reps observed that the staff files for Staff members #3, #4, #10, #11, #12, #14, #17 and #18 did not contain a signed Disclosure Statement. Cert Reps also observed that the still file for Staff Person #5's contain an incomplete Disclosure Statement that was not signed by a witness. Cert Reps observed that the staff files for Staff Member #4, #5, #11, #12 and #14 do not include verification of the childcare work experience amounts required to perform the duties of the position they were hired for. The Staff files for Staff #17 & #18 do not contain verification of either the educational requirements or childcare work experience amounts required to perform the duties of the position they were hired for.

Cert Reps observed in the STEM room that there were no emergency contact forms present for the children in care at that time.

The child file for Child #1 contained an Emergency Contact form signed 5/29/25 and the next Emergency Contact form was signed 1/21/26, more than 6 months later. The emergency contact form was not reviewed in a timely manner.
