At the time of inspection, the light cover in ceiling of the PreK 2 classroom was observed unclean with small pieces debris inside the cover. Also, peeling paint was observed on the wall near the entrance door of the Prek 1 classroom.
At the time of inspection, chipped paint was observed in the infant diapering area near the door and on the door frame. The air vent in the Braves' classroom were observed with dust and debris. A wall in the Brave's room was observed unclean. Also, chipped' paint and/or debris was observed on the window ceil in the facility.
At the time of renewal inspection, immunization record of child #1 did not include influenza immunization recommended by ACIP. A letter of exemption was not observed in the file of child #1.
