The emergency plan did not address the method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises, or a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children.
It was observed and stated that an emergency plan letter is provided to families at the time of enrollment. However, the letter observed did not contain all the facility's emergency plan procedures. The letter was missing the facility's lockdown procedure.
File for child #1 did not contain written documentation regarding the status of the child's immunizations as recommended by the ACIP and has been enrolled more than 60 days at the facility.
It was observed in several areas under and directly next to the embedded equipment in the outdoor play space that the wood mulch depth did not meet the 9 inch requirement for that type of loose fill according to the recommendations of the United States Consumer Product Safety Commission.
