The operation failed to maintain its good standing with the Texas Comp Controller and Secretary of State when the LLC was forfeited on February 21, 2025, and has not been reinstated.

Five staff files observed did not have a pre-employment affidavit available for review.

One child file observed did not have a health statement available for review.

Two staff files observed did not have the total 24 annual hours during the evaluation period, per their hire date.

Three staff files observed did not include a notarized affidavit available for Licensing staff to review.

Two children files observed did not include a health statement, and/or a parent signature acknowledging that a heath statement would be received by a health care professional within the next year.

One staff member obtained an online CPR/First-aid certification via an online course. One staff CPR/First-Aid certification did not adhere to the guidelines of skill-based training or consist of a curriculum that incudes use of a CPR manikin.

The most recent fire inspection was dated, 1/11/2024.

Based on the information obtained, one staff utilized harsh language by telling a 3-year-old child to shut up, as a result of the child crying, and/or due to not following directions. As a result of the incident, the caregiver has been terminated from childcare duties.

Ten staff files observed did not include the original certificate related to the training topic/subject received. The documents within the staff files did not include the trainer's name, qualifications, and/or the length of training.

One staff file observed contained a total of 6 out of the required 24 annual hours. Staff hire date was documented as 11/09/2022.

A sanitation inspection had not been conducted. The operation is scheduled for a health/sanitation inspection on 09/29/2022.
