Two children were missing parent signatures on the Emergency Food Allergy plan.

Several sleeping mats were torn exposing the interior padding. This was corrected during the inspection when they were replaced with new ones.

During the course of an investigation, it was determined that there was inadequate supervision in a classroom when two incidents occurred in plain sight which placed children in care at risk of harm. One incident involved children touching one another under their clothing, the other was a child being repeatedly kicked on his back by another child.

A few toy containers were cracked and sharp to the touch. This was corrected when the containers were removed from the room.

Two out of ten children's records were missing the address for the emergency contact. This was corrected when the director contacted the parents for the information.

The promotion of Physical Activity was not available for review in the Operational Policies.

A child was given a piece of fruit that was listed on his enrollment form as being allergic to it. It is unknown how much of the fruit the child ate but the child did not have an allergic reaction to the piece of fruit that day.

A two-year-old was placed in a highchair for an undetermined amount of time due to his behavior in class with the other children and teacher.

The cushion was exposed on a soft cushion age appropriate chair in the PreK 5 room resulting in it not being able to be properly sanitized. This was corrected when the chair was removed. The rug in the 2 year old room had a lot of soiled spots on it. This was corrected when the rug was removed. There was a small hole in the flooring in the older 2 year old room exposing the concrete and accessible to children and that area was sharp to the touch. This was corrected when the area was covered. There was a sharp bent paneling out on the 12-18month old playground that was accessible to children. This was corrected by placing foam over it with tape. A broken toy was found on the infant playground that was accessible to children and sharp to the touch. This was corrected when the riding toy was removed.

A transportation training was not available for review from the director's file.
