The diaper changing pad has rips in it causing it to not be sanitized and nonabsorbent. Note: A new pad was placed in the diaper changing area.

An 8 month old infant was observed sleeping in a rocking sleeper restrictive device. Note: A picture was taken for the file. A caregiver removed the infant from the device.
Four Arch climbers were observed on the playground without loose fill under or around the use zone. Note: Pictures were taken of the area. The director blocked off the area stating that the area is not used currently.
Screen time was being used with children under two years old. Note: A picture was taken and the TV was turned off during my visit.
A caregiver in charge of a group of children ages 14 mos-4 years old did not know the names and ages of some of the children.
