The second director does not have documentation that she is at this location for the required 30 hours per week.

A child file had an Affidavit Exemption from Immunizations for Reasons of Conscience in file and it was past the required 2 years from being notarized and no longer valid.

A caregiver hired on 8/1/2022 did not receive the required 8 hours of pre-service before being in a classroom and counted in the child/caregiver ratio.

A staff employed as of 8/1/2022 did not have proof of orientation in file.

Based on the evidence obtained during the investigation, parents were not notified by staff when children in care received minor injuries while in care at pick up time.

The playground checklist was documented for the month of November 2021. **This was corrected at inspection when the director completed the playground and buildings maintenance checklist during inspection.

The director was unable to provide 12 clock hours for the annual training year of 11/11/2020 - 11/11/2021.

Two children's files were missing the addresses for the individuals listed by parents as emergency contacts on the admission information form.

Two employee files were incomplete as both were missing signatures providing proof of orientation being obtained by the employees. Addtionally, one of these two files was missing proof education - high school diploma or equivalent, as well as a photo identification.

The last fire inspection was conducted for the operation on 6/15/2020. **The fire inspection was conducted during the inspection and this is in compliance.

One child's file did not have immunizations on file or an up to date affidavit for immunizations in file.
