Two outlets accesible to children under 5 were located during an inspection at the operation. The operation Director replaced the covers immediately when this was pointed out.
This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff, it was determined a child in care was left unsupervised on the outdoor playground for approximately four minutes.
The operation did not have a current fire inspection from the local Fire Marshal; the last annual fire inspection was conducted on 11/12/2024.

It was determined during this investigation that a previously employed caregiver was allowed to be present and work in direct care with children without a complete background check and Eligibility status being received. The operational policies were missing a statement regarding safe sleep for infants. This was corrected at inspection when the operation adding their safe sleep policy to their parent handbook.
