One caregiver hired after 9/1/23 did not have affidavit 2912 on file. This was corrected during inspection with the caregiver filling out the form and it being placed on file.
A caregiver did not properly supervise a child by not having awareness of the child's ongoing activities during naptime. As a result, the caregiver was not able to intervene when a child was eating several pieces of their nap mat.
Annual training in the following was needed: -emergency preparedness -preventing and controlling the spread of communicable diseases -preventing and responding to emergencies due to food or an allergic reaction -understanding building and physical premises safety -handling, storing, and disposing of hazardous materials
