AISD Lee Third Base

At inspection, all 3 caregivers were out of ratio due to staff shortage. Each of them had over 26 children. A fourth caregiver arrived during the inspection, which allowed them to be in ratio.

At inspection, two children at the operation had medication, that was not in it's original prescription packaging.

The following items are still needed out of 10 personnel files reviewed: -1 was missing an affidavit -1 was missing a diploma -4 were missing signed operational/personnel policies -4 were missing signed orientation

One caregiver was present without a background check. This was corrected at inspection when the director had the caregiver leave.

The outdoor area has a fence that blocks access to a creek. There was a hole found in the fence during inspection. A repair had been made previously, but it was no longer keeping the hole inaccessible to children. The operation stated they have put in a work order and will be keeping children away from that part of the playground until the repair is made.
