After viewing the video of Marisol and Robert interacting on the day of the incident, it was evident that Robert was grabbed and pulled into the adjacent classroom.

Caregiver did not know how many children she was responsible and did not specific ly know what age there were unless looking at the list on the board.

The operation did not allow for ECI to provide services to a child in care.

Four infants not yet ready for table food were missing monthly feeding instructions for December 2022.

Two infants under 12 months old were observed sleeping in their cribs with a blanket. Infant caregiver removed all blankets from infants' cribs during the inspection. Photos were taken and uploaded to digital storage.

During the inspection several children were napping on mats that did not match their name on the mat list.

During the walkthrough infant sink designated for hand washing was observed with a plastic container with water and a plastic spoon inside the container with an infant bib as well. Infant caregiver confirmed bottles and infant utensils are usually washed in the sink.
