One new caregiver was missing a notarized affidavit for employment and a copy of her photo ID. Two new caregivers were missing documentation of orientation and proof of education.
Four caregivers were missing training on some of the required annual training topics for their last completed training years.
It was found that a caregiver used her hand to thump children on the head. It was also found that the caregiver raised her voice at children in a harsh manner.
An incident report was not filled out after an incident in which a child was left unsupervised for a few minutes.
A child was able to leave the building and was unsupervised for a few minutes.
