A review of the Employees Who Left Employment in 2025 survey conducted January 20, 2026, revealed the operation failed to report the number of employees who left employment in 2025 by the January 15, 2026, deadline.

Two children out of 10 children's files evaluated did not have current immunizations.

Four children over the age of four years did not have a vision and Hearing screening completed. Two children turned 5 years this year, one child turned 5 years in 2024, and one child turned 4 years September 2024.

An employee without a background check was present at the operation with access to children.

This standard was evaluated and found to be out of compliance when a caregiver placed a child in a restrictive device to discipline the child.

A child was in the hallway bathroom alone. The caregiver was in the classroom with the rest of the children.

The operation added school age care prior to notifying Child Care Regulation. This was corrected at inspection when the director informed Child Care Regulation of the added service.

A child with a sever food allergy does not have an allergy emergency plan on file.

Daily activity schedule is not posted in the classrooms for each age group.

A child's severe food allergies was not posted in the kitchen where food is prepared.

One child does not have a record of immunizations in the file.
