Two children had allergy action plan that were not signed by both the parent and the medical professional.

The garden and fence had sharp edges and nail that were accessible to children.

All required training courses where not available for review. Training hours were sufficient but did not include the required topics.

Employee files did not contain all of the required information.

One infant had a bib inside of the crib with her and had a long toy sitting over the top of the crib on the frame of the crib while the infant was inside of the crib asleep. A second infant was alseep with a bib on and had a small toy inside of the crib with him. The items were removed from inside of the crib and from the top of the crib during inspection.

Two infants were asleep in restrictive devices. The infants were moved into cribs by a caregiver during inspection.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Information obtained throughout the investigation shows that a caregiver admitted children to care before receiving the required admission information.
