The infant feeding instructions were not updated for the month of June.

One child's prescribed medication was found to be expired as of November 2023.

There was a bottle of disinfecting spray on the floor in an area used by children. It was moved during the inspection.

Two caregivers stated they only use soapy water to clean the diaper changing surface.

Two staff members stated that they have been changing infant diapers directly on the carpeted floor.

Four caregivers did not have a notarized licensing affidavit for applicants for employment.

The operation did not have their license posted.

The toddler classroom had one teacher to 12 children making them out of ratio. During the inspection one child was moved to another room making both classrooms in ratio.

The most recent fire inspection was not avaialble for review.

Out of 11 infants enrolled 6 did not have up to date written feeding instructions.

The fence in the toddler play area has peeling paint that is accessible to the children.

Cleaning supplies was stored under sinks without child locks having it be accessible to children. *This was corrected at inspection when the cleaning supplies and removed and placed out of reach of the children.

Out of nine staff employeed three files were not available for review.

The child's action plan was reviewed. The doctor signature was on the form and the date was missing. The parent's signature and date was missing from the form given permission to give medication.

The child's medication records were not available.

The operation has failed to follow up within 7 days after an employee was no longer employed at the operation. Th caregivers last day at the operation was 3/11/2022 according to the director. According to the on line background check, the caregiver was listed employed at another operation as well as Busy Bee Academy. The caregiver was inactivated during the today inspection and validated on 4/1/2022.
