A caregiver is missing 15 clock hours of the required annual training for August 14, 2024, through August 14, 2025. The caregiver must complete 15 clock hours of make-up training, which will not count toward the current training year.

CCR reviewed the files of four children, and one child is missing the physician's information on the admission form.

A total of 14 out of the required 20 annual training hours were observed. Training topics observed included: age appropriate activities, child maltreatment, bloodborne pathogens, child development, positive guidance and discipline, safety precautions, emergency preparedness, and administering medication.

One out two employee files did not contain a signed and dated statement for orientation, and/or pre-service. One caregiver did not have a personnel form avaible for review in order to verify specific information.
