During the inspection the first aid kit did not contain a thermometer.

Five employees files were reviewed and missing the proof of education qualification during the inspection.

One caregiver did not have a copy of their photo ID in file while records were being reviewed.

I observed that the first aid kit did not include a guide to first aid.

I did not observe documentation of current certification in first aid in 1 out of 2 personnel records evaluated.

I did not observe documentation of a personnel file for a caregiver who was observed working with children during the inspection. I observed that one personnel file was missing the following: 1. Notarized affidavit 2. Documentation of orientation 3. Documentation of a date of hire 4. Documentation of High school diploma

I did not observe a date of employment documented in 3 out of 4 personnel records evaluated.

I did not observe documentation that a caregiver had received orientation in 3 out of 4 personnel records evaluated.

I did not observe documentation of pre-service training in 1 out of 4 personnel records evaluated.

I observed that the employee affidavit had not been signed or notarized in 1 out of 4 personnel records evaluated.

I observed the following while evaluating children's enrollment records: 1. An address was not documented for the emergency contact person in 1 out of 5 children's records evaluated. 2. The name of the elementary school was not listed in 2 out of 5 children's records evaluated.
