It was found during the inspection that an infant under 12 months of age was sleeping with a pacifier that had a stuff animal attached to the pacifier. This was corrected during the inspection by removing the pacifier and stuff animal from the crib.

During the investigation a caregiver admitted to using their cell phone for personal reasons while supervising children.

It was found during the investigation that a caregiver admitted to restraining a child during naptime to force the child to sleep.

It was found during the investigation that a caregiver inappropriately disciplined a child.

It was found during the investigation that caregivers engaged in a physical altercation in the presence of children.

2 out of 7 children's records reviewed was missing an updated shot record verifying they are are current on all immunizations.

During the inspection it found that a caregiver did not know how many children they were responsible for. Caregiver had to verify the number of children by looking at their sign in/out sheet.
