Three out of ten staff records did not have the required trainings complete at inspection.

At the time of inspection the caregiver in the school age classroom walked through the four year old classroom and out the front door of the building, leaving the children in the school age classroom unsupervised.

The young infant room floors and walls need to be mopped and wiped down.

At the time of inspection there were six children ages 12 to 17 month combined with five children age 18 to 24 months old for nap time with one caregiver. This was corrected when the 12 to 17 month old children were placed in a classroom separate from the 18 to 24 month old classroom.

There were not any activity plans posted for four classrooms.

Based on the information gathered, a child was placed in time out for more than thirty minutes.

Based on the information gathered and observations made, the caregivers are yelling at the children in care.

Three nap mats in the 18 to 24 month old classroom were ripped and the foam was protruding. This was corrected when the nap mats were replaced.

Based on the observations made, there is not enough toys and activities in the young two year old classroom and the 12 to 24 month old classroom.

Two employee were working in the center without a current background check and current FBI fingerprints.

At the time of inspection the 18 to 2 year old caregiver yelled at the children in care.

At the time of inspection, the 18 to 24 month old caregiver was observed roughly grabbing children by the arms and placing them on the carpet roughly.

At the time of inspection, there 18 to 2 year old class was watching Mickey Mouse Club House. This was corrected when the television was turned off.

Based on the information gathered the children are not being adequately supervised as there are a lot of bites happening each day.

The sheet rock behind the toilet in the boys restroom in the second building, was coming away from the wall. The baseboard in the girls and boys restroom was coming away from the wall. The bolts holding the toilets to the floor are too long.

The activity plans were not posted in the classrooms at the time of inspection.

At the time of inspection the two year old children in both two year old classrooms did not go outside for the scheduled morning outdoor play.

Two outlets in the preschool classroom were not covered. This was corrected when the outlets were covered.

At the time of inspection there was one medication that was not in its original container with the child's name on it.

The bolts holding the toilets to the floor are too long. There are broken pieces of plastic on the playground accessible to the children in care.
