It was determined through the course of the DFPS investigation that a child was left in a bouncer for more than 15 minutes.

A trash can containing soiled diapers was observed without a cover, making the contents of the trash can accessible to children in care. This was corrected at inspection when the garbage was removed and made inaccessible to children.

A caregiver in the 3 year old classroom was present at the operation before receiving notice from the CBCU regarding their eligibility.

1 of the 10 children records surveyed did not have current immunization records.

During the inspection one employee file could not be located for a new hire. Of the 10 personnel records surveyed, one employee did not have proof the GED/high school diploma.

The rotating menu utilized by the operation did not reflect substitutions utilized for PM snack on 8/11/22. The Texas Department of Agriculture paperwork also reflected inaccuracies reflecting food served to children in care for PM snack. Substitutes were not indicated on either report.

An infant was observed to be sleeping in a restrictive device. The infant was moved to a crib during inspection.

One child who had a note on a doctor's prescription pad paper instructing not to give the child a certain food did not have a food allergy emergency plan on file.
