During the monitoring inspection, medication for children enrolled at the operation was observed. There were two children's Epi Pens that were expired. One Epi Pen expired 8/25/2025 and the other Epi Pen expired 11/2025. I spoke with the director about sending the expired medication home with the childrens parents.
One caregiver was present with children before receiving a background check eligibility letter. This was corrected during the inspection when the caregiver left the operation.
There was no documentation of caregivers completing hazardous material training.
The operation could not provide the most resent copy of the sanitation report.
During the monitoring inspection it was found that one of the staff files reviewed did not have a notarized affidavit available for review.
