The diaper changing mat was observed with crumbs on it. This was corrected at the inspection when the caregiver cleaned and sanitized the mat.

In the toddler classroom, the ceiling tiles were observed with dust. A hole was observed in the ceiling of the restroom. Paint was observed peeling from the ceiling of the toddler bathroom.

Two of the classrooms had outlets that were missing covers. This was corrected during the inspection when the outlet covers were replaced.

During the inspection it was observed that 3 infants: 6 months, 9 months, and 10 months, were sleeping in a bouncer, which is a restrictive device. All 3 infants were also sleeping with a blanket.

During the monitoring inspection it was observed that there was a group of children playing on the playground and they didn't have any water available to them.

During the inspection it was found that the monthly fire drills were not being documented.

During the investigation inspection it was observed that there were two spots in the hallway where the ceiling was not in good repair. There was evidence of water damage and holes in the ceiling.

A review of the Employees Who Left Employment in 2023 data from January 18, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.

During the monitoring inspection it was observed that there was no loose fill under the spring rockers on the playground.

During the inspection it was observed that there was an caregiver present at the operation with direct access to the children before the operation had received notification from the CBCU that the subject of a background check was eligible to be present. Caregiver left operation while Child Care Regulation was present.
