Ten of ten children's records did not have addresses for emergency contacts.

Six foot use zones were not maintained around playground equipment. This was corrected when the director instructed caregivers to reorganize the playground equipment and remove toys from fall zones.

The rooms did not have posted dated written activity plans.

The operation did not have a current fire inspection report. Last on file was 07/12/2021.

One caregiver in Room 6 was using her cell phone at the table while supervising children who were napping. This was corrected when the caregiver put away her cell phone.

Room 3 was too dark to visibly see the children in the classroom during naptime. This was corrected when the blanket was partially removed from the window to provide light.

A child was sleeping positioned directly in the exit path of the classroom doorway. This was corrected when the child was removed from the exit.
