A caregiver without a cleared background check was caring for kids. NOTE: Caregiver was removed from classroom
The caregiver turned away from a child that required supervision during meals. This resulted in a lack of supervision as the child was able to reach for a cup of milk not assigned to them. The child was able to place the cup of milk against their lips and it is alleged child consume the milk due to the child’s severe allergic reaction symptoms.
Based on information gathered during investigation, staff failed to attend a child with a severe allergic reaction to milk. Staff was aware child placed a cup of milk to their lips and only removed the object, but did not rinse the child’s mouth or monitored the child for symptoms. Staff neglected to use good judgement towards the evaluation of the child and lack of communication with staff.
Based on information gathered during the investigation it was found that a person was present with children in care prior to the operation receiving a notification letter of eligibility.
The transportation van used for field trips purchased in 2015 does not contain a child safe alarm. Corrected at inspection: The center will no longer be conducting field trips.
Based on evidence obtained caregiver failed to communicate information regarding a child's severe allergies with the upcoming caregivers to continue care.
The operation did not have an allergy list posted in the kitchen that listed the allergies for the children in care.
The operation reported Covid eight days after the first positive case was confirmed which is outside the required two day timeframe.
