A review of the Employees Who Left Employment in 2024 data from January 17, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
During the inspection it was observed that there was an individual present at the operation, with direct access to the children in care, without a clearance from CBCU that they were eligible to be present.
During the investigation it was found that a child was left alone without any caregivers supervising them for at least 23 minutes on 11/12/2024. This was cited. Supervision was observed during the inspection to meet compliance.
The director first aid training expired 12/13/2020 and the HISD teacher did not have first aid training.
There was sufficient evidence that this event occurred.
