Observed chipped/peeling paint from the wall in several classrooms. The rubber/vinyl stip around the wall base has come lose in one of the restrooms down stairs and one restroom upstairs. A John Deere tractor on the playground is rusted and the wooden boards in the equipment exhibit rough, unsmoothed edges, presenting a potential splinter hazard. Ceiling tiles were observed in one classroom with water stains and need to be replaced. . A fan with an rotating blade was observed in one classroom with the sitting on a piece of furniture with the electrical cord hanging down accessible to children posing a risk of the children pulling the fan off the furniture. The caregiver removed the fan from the children's access duringinspection. A phone charger plugging into a charging brick was observed on a restroom sink counter top. The phone charger was not connected to an electrical outlet. A caregiver removed the charger from the restroom sink counter during inspection. The bottom vent is not properly attached to a refrigerator in one room used by the children. A staff member attached the refrigerator vent at inspection. Two large limbs were observed on the playground and need to be removed.These were removed from the playground during inspection. Large plastic pieces were observed on the ground on the playground between the entrance of the infant playground and older children playground that present as tripping hazards. These were removed from the playground during inspection. A garden hose and sprinkler equipment were observed on the playground and pose as a tripping hazard. The garden hose and sprinkler equipment were removed from the playground at inspection. A red plastic cover was observed hanging from a portion of a side of the fence around the a/c unit with excess material on the ground creating a tripping hazard. Two medium sized pieces of wood were observed on the playground posing as a splinter hazard. A sleeve of foam drinking cups were obseerved on the ground of a Gaga pit on the playground. The red plastic cover, two medium sized pieces of wood, and sleeve of foam drinking cups were removed from the playground at inspection.

This standard was reviewed on 5/6/2026 as a follow-up to a previous inspection and found deficient. A piece of wooden equipment was observed on the playground with broken wood posing as a potential splinter risk. A small piece of metal equipment on the playground with metal chains. It is not anchored in the ground and is not steady as well as has rusted chains and needs to be removed. A wooden tables stretcher that connects the table legs horizontally on one side of the table has broken making the table unsteady and needs repaired or to be removed from the playground.A standing outdoor sand box has collapsed and needs to be removed. Wooden item was observed on the ground on the playground with protruding screws and is posing as a riping hazard. The latch on the gate on the fence around the a/c unit on the playground is missing and need repaired.
