There was a caregiver did not know the number of children in their care when asked.

A child was alone in the hallway for less than a minute unsupervised by a caregiver. During the investigation, caregivers were not able to indicate how nor at what moment the child left the room.

Medication was found above the diaper changing table in the 2-year-old room accessible to children. This was corrected when the director removed the medication from the room.

One employee was working in the center in direct care of a group of children without a current background check and current FBI fingerprints.

Based on the information obtained, a child was left unsupervised in the bathroom in the classroom.

Based on the information gathered the children were in the activity center when one child left the activity center unsupervised and walked down the hallway towards the classrooms.

A child was left unsupervised in the gym area.

Four medications were expired at the time of inspection. This was corrected when the medication was discarded.

Two medications were not in the original container.

Upon arrival to the center, there was not a person in charge. This was corrected when the center director arrived at the center.

Based on the information gathered a child was left alone in the classroom, unsupervised for more than a few minutes.

During the inspection, one child was walking down the hall unsupervised. This was corrected when the child returned to the classroom with the rest of the group.

One caregiver involved in the incident was not certified in First Aid.
