On the outdoor play area, the use zone in front of a slide overlapped the use zone of another slide. Pictures were taken. Deficiency was corrected at inspection when the director repositioned the slides to reflect proper use zones. Pictures taken.

The operation did not have current infant feeding instructions for a child. The last feeding instructions expired in July 14 of 2022. This was corrected at inspection after parent/custodian of child provided the operation with currently signed feeding instructions dated 8/3/22.

During inspection, in the outdoor play area, a piece of play equipment with plexiglas was partially broken, exposing a pinch/cutting point. Photos taken. This was corrected at inspection after the partially broken equipment was repaired with industrial duct tape, no longer presenting a hazard. Photos taken.

The operation did not have the list/form with the emergency telephone numbers posted. This was corrected at inspection after the operation completed the emergency telephone numbers form 7243 and was posted.
