After being aware of a child who was able to open the playground gate with ease on more than one occasion, immediate action was not taken to better secure the gate. Video of an incident was observed where the child opened the gate and ran before being retrieved by the caregiver on 3/25/26. At the time of the investigation inspection on 4/2/26, it was observed that a correction to better secure the gate had not yet occurred, which showed continued risk.
Fire drills were not conducted for September and December of 2025.
The director did not have current training in SIDS, Shaken Baby Syndrome and Early Brain Development.
2 children's immunization records did not have documentation of all required immunizations.
While redirecting a child's behavior, a caregiver grabbed onto a child for approximately 44 seconds.
Care being provided was not consistent enough to address a routine occurrence of aggressive biting by a child in care. As a result, several children were being bitten on a routine basis, sometimes causing bite marks that lasted at or longer than 24 hours.
A caregiver did not use good judgment or competency in how they communicated with a parent after their child received an accidental injury that was above minor. The parent was provided with insufficient detail of the injury and did not indicate how serious the injury was. As a result, the parent was not able to accurately access their child's injury and that it needed more immediate attention than waiting until pick up time. The child had several abrasions across the face, a bruise and later began to display signs of a possible concussion prior to being picked up by the parent. The caregiver also did not ensure that the director, who was still present at the time, was properly notified. The director was not made aware of the incident until after the child was picked up by the parent.
A caregiver did not comply with minimum standard rules regarding proper communication with the director and parents for more serious child injuries.
3 newly installed pieces of active play equipment had overlapping use zones.
Child admission records did not have verification that parent's received a copy of their parental rights.
An active play structure at the playground did not have the required 6 feet of use zone in all directions. The use zone was corrected during the inspection.
A child was not accounted for and was left in the classroom between 5 to 10 minutes when the caregivers took the children to the playground.
One child who also attends public school did not have the school name or phone number included in the child's file.
The operation did not have a current annual fire inspection.
A small outdoor play structure with steps and a slide did not have loose-fill material, as required.
